SUMMARY:

Considering the need for non-invasive, accurate and risk-free identification of patients with mammary gland tumors with the possibility of lymph node metastases, this study aimed to evaluate and compare the accuracy of B-mode, Doppler and compression elastography techniques for the diagnosis of inguinal lymph node metastases in female dogs with mammary neoplasms. The study was carried out at the Veterinary Hospital of UFRA, for selection of animals. Each lymphatic structure was evaluated with B-mode ultrasound, compression elastography and color Doppler methods. The elastographic images were evaluated in a semi-quantitative and qualitative manner, resulting in images with a color scale (elastogram) that were evaluated according to their deformity, with reddish tones corresponding to less rigid tissues (soft) and bluish tones to more rigid tissues (hard). A total of 23 pairs of inguinal lymph nodes (n=46) were collected after surgery, followed by removal of the mammary chain from each animal to identify metastases and classify the tumor type. Of the 23 animals that underwent mastectomy, 19 types of malignant neoplasms and 4 benign ones were detected, and of the lymph nodes collected, 19 already had metastasis. When evaluating the lymph nodes in b-mode, there was a statistical difference only in relation to the size and shape of the lymph nodes (P<0.05). In Doppler mode, there was no statistical difference, although the location of the vascularization within the lymph node showed a strong association with the diagnosis, with a tendency for the vascularization to be hilar in non-metastatic nodes and to be of the peripheral or mixed type in the metastatic group. The distribution of elasticity scores was significantly different between groups (P<0.05), with scores 1, related to soft elasticity, being more frequent in benign nodules and scores 4 and 5, related to rigid elasticity, being more frequent in malignant nodules. The results obtained in our study support the use of the techniques as noninvasive screening diagnostic tools in oncology patients to obtain additional relevant information that will help in determining treatment and prognosis.
